for me there are some common risk that can be associated when an IS/IT is being changed.
1. minor malfunctions of the system(implementation) - when a certain system is change, of course the practice will also be changed. thus, there will be a tendency that the implementation of the system will encounter some difficulties in letting the people adapt the change in the system.
2. loss of data - there will be a tendency that during the system change some data will be lost at hand while the operation of the change is ongoing. that is, if the people responsible in changing the system is dumb enough to oversee some things and commit mistakes in the work.
3. mistakes in operation - when a new system is implemented, there will be a tendency that people will not easily adapt the system even for the people who will be served by an institution. a lot of people involved in the new operation will not easily get used to the system.
4. temporary loss of integrity - trust will not be established between people who benefit from the system for it is still new, doubts in making transaction under the system will be common but only temporal.
5. commonly committing mistake - if the people are new under the system, people operating the system might commit mistakes while operation is being done.
